All referees must submit a Verification Form signed by the coaches.  This is a new style form with 3 games per form. The old style forms are still accepted. Single games may be submitted on the three-game form.

Please do not staple forms or tape envelopes closed. Send completed, signed forms by US mail to:

If you would rather drop your forms off to the bookkeeper, you must email her (the first time) at acc.referees@live.com

You may instead scan in your signed forms and email them to the bookkeeper at mailto:acc.referees@live.com. PDF is preferred and you MUST put your first and last name in the email subject line. Multiple page documents are fine. The scanned, signed forms must be completely filled in as usual.

• Pay is for properly licensed referees. Club lines and non-licensed referees are not paid.
• Referees failing to complete all paperwork and reports are subject to forfeiture of pay.
